• PEMG will process all insurance claims and/or billing procedures on your behalf when and if  your union/employer is a client of our and/or if PEMG participates in your managed care provider program (Please call PEMG to verify participation in your plan)

• PEMG will check eligibility prior to your examination date.  If you are not eligible, PEMG will make every "good faith" effort to contact you.

• Some managed care plans require a co-pay.  You are required to bring the co-pay with you at the time of your exam.

• For medical tests requested outside your insurance coverage, you will be required to pay cash or check.  PEMG does not accept credit cards.

Insurance & Reimbursement

Professional Evaluation Medical Group will check eligibility and process all insurance forms on the patient's behalf.  Insurance reimbursement requires that the periodic preventive physical is covered in a members benefit plan.  We accept several insurance plans including the following:
