In
order to facilitate the appointment procedure, prior to scheduling an
examination by telephone, you should have the following
information:
- Name,
address, telephone number, date of birth of the patient being
scheduled.
- Insurance
card, with the name of the insurance company, and the Social
Security number of the member/employee covered by the insurance.
- Primary
insurance information of dependent/spouse if covered by another
insurance company.
- All exams
require prior scheduling. There are no walk-ins for the
Annual Physical exam.
Your
Customer Service Representative should, at the time of your call:
- Be
courteous, and provide and answer to any questions you have about
your exam process.
- Schedule
you and/or your family members at the time of the phone
call. PEMG will try to schedule your appointment within a
two to four week period.
- Will
remind you of a co-pay requirement, if applicable. To verify
a co-pay requirement, please contact your insurance carrier for
more information.
- PEMG will
check your insurance carrier/union/company for eligibility prior
to your examination.
PEMG
examinations are conducted on weekdays and weekends, primarily in the
morning in order to accommodate the fasting requirements for patients
having bloodwork.
Please
show up for your scheduled exam time. If you cannot make
your appointment, please call PEMG during business hours (Monday
through Friday, 9:00am to 4:30pm) prior to your exam to
reschedule.
